A research letter published Oct. 26 in JAMA found that in the first six months of Medicare's Wasteful and Inappropriate Service Reduction (WISeR) pilot, use of targeted services fell in participating states relative to the rest of the country. Researchers from the University of Minnesota School of Public Health and Brown University reported a drop of nearly 20% in some services, as covered by Becker's Hospital Review and Becker's Payer Issues.
Do this week. Pull your own volume and authorization data for WISeR-targeted services in participating states and compare it with the JAMA findings before your next payer review. Run this
- The evidence.
- The JAMA research letter, as reported by Becker's, links the pilot to a nearly 20% relative drop in use of some targeted services over its first six months.
For the Chief AI Officer. Brief the board that AI-enabled prior authorization now has a measured effect on service volume and add WISeR exposure to the AI risk register.
For the CFO and revenue cycle. Model the revenue effect of a relative volume drop on WISeR-targeted services in participating states and adjust forecasts.
For the Chief Medical Officer and physicians. Ask physician leaders to review whether care in targeted services is being delayed or forgone and document clinical rationale for appeals.

On The Robot Report Podcast, executive editor Steve Crow quoted GXO's Patrick Kelleher: 45 humanoid pilots so far and no ROI on humanoids yet, a couple of years away. Senior editor Mike Oitzman cited Agility Robotics CFO Michael Beer on a bill of materials of roughly $150,000 per robot at around a thousand units a year, possibly falling to about $30,000 at 10,000 units. Oitzman also reported A3 Q3 North American robot orders up 24.8% in units, with life sciences among segments showing gains.
Ask your team. Ask your operations team what return a humanoid or mobile robot pilot would need to show before you would fund one. Run this
